CHART - How to Contact Us

Referrals

CHART accepts referrals from professionals and carers with service user's consent. Although self-referrals can be accepted it is preferable a referral agency to contact us. Referrals are discussed and allocated each Monday morning.

The CHART service is available for all housing and support providers to contact for accommodation advice, support and signposting.

Our referral form can be downloaded here and can be emailed to ChartTeam@crosby-ha.org.uk or posted to CHART, 10 Church Road, Waterloo, L22 5NB

CHART uses the Crosby Housing Association complaints procedure. As such, any complaint will be dealt with speedily and at a local level.
