Community Housing and Re-enablement Team (CHART)

The CHART team works with Sefton residents who are in contact with adult mental health services.

They have a focus on meeting the accommodation needs of service users who are experiencing, or have experienced, a severe and enduring mental illness.

The team works with homeless, and potentially homeless, people with mental health problems, particularly those whose discharge from hospital is being delayed.

They also work with those who are living independently in the community yet have an accommodation issue which is impacting negatively on their mental health.
